This grant supports promotion of wellness and chronic disease prevention through state-wide and local policy environment and environmental change. Specific areas that are addressed through this grant are obesity, physical activity, nutrition and tobacco prevention and control. This funding was provided to implement evidence based prevention and wellness strategies that support specific measurable health outcomes to reduce chronic disease rates that achieve the following outcomes: • Increased levels of physical activity • Improved nutrition (e.g. increased fruit/vegetable consumption, reduced salt and trans fats consumption); and • Decreased exposure to secondhand smoke.

Quarterly Activities/Project Description Nutrition: The Provider met with three food service management companies to identify and address anticipated systematic barriers to implementation. The Provider identified existing relationships with approximately 77 schools who independently manage their food service and effective communication strategies to recruit them to participate in the nutrition initiative. Food labeling system signage and food labels were provided to Metz Associates (food service management company) to be distributed to pilot schools for implementation in late April. The Grantee worked with Metz Associates to identify the 20 schools to participate in the nutrition pilot. Physical Activity: The Provider delivered a technical assistance workshop in order to train community-based project coordinators and walking group leaders in the development and implementation of walking groups. A walking group leader manual was developed and distributed to the identified walking group leader in each target community. The Provider identified types of signage to be used to mark local walking routes. The design for the signage was completed. The number and types of signs to be purchased were determined. Print and electronic materials were designed to promote walking routes and groups. The Provider received maps of all 27 proposed walking routes from six target counties. Tobacco: During this quarter, in the 18 target counties 16 volunteer community organizations and 154 people were trained in Young Lungs At Play (YLAP). There were 26 community presentations made to local governments, and 149 policy makers were contacted. There were 10 youth organizations advocating in communities (Teens Against Tobacco Use groups, high school students, Leos Club, school districts, 4-H Clubs) and 16 additional ordinances were passed this quarter, for a cumulative total of 47. A total of 110 parks and playgrounds have been designated tobacco free; and signs are pending installation.

Description of Jobs Created Administrative Officer 2: Provides administrative, fiscal and budgetary support. Responsibilities include development and updating of reporting templates; monitoring contractor compliance with the reporting requirements to ensure proper submission of required reports. Administrative Assistant 1: Provides administrative, fiscal and budgetary support. The responsibilities include performing a full range of administrative services, including assistance with budget preparation and management, contract/grant administration, invoicing, personnel, purchasing and procurement, data processing, and publication/ report preparation. Principal Investigator: This position is responsible for contract administration and accountability as well as advising the project team on best practices. Community Project Director: This position is responsible for the design and implementation of the project, identifies community-based partners, and directs the work of the project team. Project Coordinators: These positions are responsible for executing tasks associated with the day to day operations of the project, including scheduling and attending meetings; communicating with subcontractors; and monitoring subcontracts with community partnership organizations. Communication Coordinator: This position is responsible for the design and development of the print and electronic promotional materials, including the web design. Financial Administrator: This position implements and monitors the project budget. Faculty Content Experts: These positions are responsible for identifying research linking physical activity to chronic disease and aging, evidence-based and promising practices for policy and environmental change, and gaps in the literature; recommending strategies for policy implementation; and designing the evaluation plan, logic model, and data collection instruments. Community Project Director: This position is responsible for overseeing activities to assure timely completion of project deliverables. Community Communication Specialist: This position is responsible for project coordination, including identification of local partners, identification and mapping of walking routes, convening walking groups, and facilitating promotional events. Student Worker: This position is responsible for the development of the policy brief and identification of local policies related to pedestrian transportation. Writer: This position is responsible for writing promotional and collateral printed and online communications. State Project Coordinator: The State Coordinator position provides training, technical assistance to county Program Assistants, schedules, collects, and evaluates monthly reporting. Part Time Assistant: This position coordinates the distribution and collection of monthly reporting and provides assistance to State Project Coordinator. Program Assistants: These positions identify appropriate contacts to promote and implement this program in their respective counties. The Program Assistant reports monthly activity to the State Project Coordinator and disseminates approved signs and materials to local municipalities and governments implementing the program. Principle Investigator: Responsible for overseeing implementation and working with the PA Department of Health to accomplish the nutrition component milestones, from working with PA food manufacturers to identify healthy foods and beverages and creating and maintaining relationships with food service management companies and schools to working with partners, such as the Department of Education on nutrition analysis. Faculty: Faculty will assist with required research, development and evaluation for the nutrition component, such as researching menu labeling systems for school cafeterias and vending machines to designing the measurement standards to verify labeling standards and data collection.
