This application addresses broad Challenge Area 15-MH-105, Strategies to support uptake of interventions within clinical and community settings: Develop and pilot comprehensive implementation strategies to support the broader uptake of interventions within clinical and community settings.
Drawing upon behavioral change science and distance learning methods, this
award Enhancing Pediatric Psychopharmacology in the Medical Home will test
the relative impact of two interventions of differing intensity, both designed to increase
primary care providers, (principally physicians), use of expert-recommended best
practices in the assessment and treatment of children and adolescents with ADHD.
This study of strategies to change physicians' ADHD practices addresses an area of
urgent public health need and keen interest, given concerns about ADHD under- and
over-treatment, potential side effects of the drugs used to treat ADHD, and the failure to
employ other interventions that might be required to optimize child and family outcomes.
Importantly, findings from the NIMH Multimodal Treatment of Children with ADHD
(MTA) Study (1999) indicate that carefully medication management is significantly more
effective and cost-effective than usual care, yet available evidence indicates that most
"real-world" primary care clinicians have not incorporated these findings into their ADHD
management practices. This application capitalizes on these new findings to address
this area of great public health impact. Moreover, because basic behavior change
methods and learning theory are incorporated into the interventions, findings from this
study will be of great interest to other scientists and policy-makers struggling to close the
research-practice gap.

1. Research Assistant 1 assists training coordinator in scheduling the bi-weekly consulting telephone conference calls with participants, attend calls, and prepare first drafts of meeting minutes from all biweekly consultation calls for review and approval of the training coordinator. Under the guidance of the Principal Investigator, she completes follow-up semi-structured debriefing interviews with participants at the end of each of the 6-month trainings, in order to obtain more qualitative information about the benefits of the training and changes in PCC's practices. She assists the Training Coordinator with packaging and delivery of materials, and compiles overall feedback reports based on evaluations.
2. Research Assistant 2 works under the direct supervision of the Consortium Principal Investigator. She assists REACH in handling logistics and scheduling for the training meetings and twice-monthly consultation phone calls, process all subject payments and travel reimbursements, assists in the contacting the office staff of 30 PCCs receiving the enhanced ADHD training program, handing their logistics and travel, and assists the Consortium Principal Investigator in contacting and scheduling the training of the 30 case managers. She mails and collects all research questionnaires and medical records data from participating PCCs at each of 3 assessment points and then enters data into an Access database overseen by the Consortium PI.
