To continue to provide comprehensive health care services to a population that rarely seeks medical services and to augment primary health care services, by provide at least three specialty clinics each month for a 24 month period. Specialty clinics may include but are not limited to: health screening, colon cancer screening, general physical exams for men and women, immunization clinics, PSA labs for men over 40, diabetic eye and foot clinic, nutrition clinics, mental health clinics (grief counseling, etc.) and dental clinics for prevention
Improve the medical, social and education outcome for parents and young children by establishing a schedule of parent education workshops focusing on immunizations, healthy baby screening, health diets Increase access to more well child services Develop a monthly schedule of services and promote throughout the community, schools and childcare centers.
To increase enabling services to new and established patients, through enhanced outreach and transportation. We aim to increase access to care for seniors and patients with limited mobility and/or a limited mode of transportation, by utilizing the already established van service Increase the visibility of clinic services throughout the service area by having an outreach worker to field questions, handle eligibility, and facilitate networking for specialty care
IDS funds will also allow us to continue with IT support for providers and other medical staff using the new EMR program. This support will assist providers in becoming more proficient in the EMR hence allowing for more visits during the day
