We now apply for an Administrative Supplement to obtain such funding for each of 2 years, to obtain additional clinical material from Dr Swisher which is needed to evaluate mesothelin as a biomarker for immunodiagnosis and monitoring of patients with ovarian carcinoma and to explore two new leads from recent studies supported by the parent grant. We expect that an award would lead to better ways to diagnose and treat OvC and yield both short-term and long-term economic gains.

Quarterly Activities/Project Description 1.The manuscript "Autoantibodies to mesothelin in infertility" by JL Luborsky, Y.Yu, SL Edassery, J Jaffar, YY Yip, P Liu, KE Hellstrom and I Hellstrom has been published in Cancer Epidemiol Biomarkers Prev 20 (9), 1970-1978, 2011. Epub 1011 Aug 16.2. A manuscript "Evaluation of ovarian cancer remission markers HE4, MMP7 and mesothelin by comparison to the established marker CA125" has been submitted to Gynecol. Oncology by M Schummer, C Drescher, C, R Forrest, S Gough, J Thorpe, I Hellstrom, KE Hellstrom, and NE Urban, N. E. Our conclusion is that HE4 can predict ovarian cancer recurrence earlier than CA125 and be elevated in patients that do not express CA125 at sufficient levels to make a decision. MMP7 and mesothelin have lower potential as markers for ovarian cancer recurrence to complement CA125. Elevated HE4 throughout remission could indicate poor prognosis.3. We have used the Quiagene's Expression Kit E. coli for high level expression of mesothelin in E. coli to provide a potentially better source than CHO-produced mesothelin for antibody and T cell assays. So far, we have tested a large number of control sera/plasmas to establish cut-offs and will compare E coli produced mesothelin with the COS-produced material. 4. Ascites from patients with advanced ovarian carcinoma have an abundance of immature APC. We can mature these cells by culturing with magnetic beads conjugated to Mabs to CD3 and CD28. After maturation of the APC and in the presence of the patient's tumor cells, the supernatants contain large amounts of IFNgamma and TNFalpha, and we have preliminary evidence that specific T cell responses to mesothelin can be generated.
