STONY CREEK COMMUNITY HEALTH CENTER INC


Stony Creek Community Health Center (SCCHC) has operated from a single site since 1989. Through the ARRA CIP grant, SCCHCs goals are (1) to enhance patient handicap access, (2) improve clinical diagnostics and patient outcomes, and (3) increase patient safety. The project includes a building alteration with two electronic doors openers to facilitate safer and easier access for wheelchair bound and handicapped patients. Additional lab equipment (hematology ACT diff blood analyzer and I-stat analyzer) will promote faster diagnosis, treatment and improve patient outcomes; upgrading to digital x-ray will improve the quality of the x-ray for more accurate reading; and replacing exam and waiting room chairs will provide more comfort, safety and better infection control for patients. The importance of achieving these goals to improve our record of being a patient centered medical home to our patients by providing exceptional care in a well equipped, modern office setting.

Quarterly Activities/Project Description Hematology ACT diff blood analyzer and I-stat analyzer has been in use since the first of 2010. The doctors and patients have both benefited from the in-house hematology lab equipment; the convenience of almost instantaneous results has allowed the physicians to have immediate dialogue with patients regarding diagnosis and treatment options. Electronic door openers were installed in September, 2010 and have been well-received by patients, especially those who are elderly and/or handicapped. Staff provided instruction to patients on operation. Patients appreciate the ease of use and that they are able to enter or exit without assistance, giving them back a measure of independence. Digital X-ray was installed April 13, 2011. Doctors are very pleased with the clarity of the x-rays. We are also able to burn an X-ray CD for patients that we refer to a specialist, saving them the cost of an x-ray by that provider. During June, 2011, waiting room and exam room chairs have been replaced with bariatric chairs (can accommodate patients weighing up to 400 lbs.) which are sulfide/ urine resistant and stain, moisture, bacterial resistant. This is the final report. There is an unobligated balance of $119.58. The reason for this unobligated balance is that the cost estimates for equipment made at the time of the application differed from the actual cost. Two of the five items cost more and three cost less. All items detailed on the application were purchased and currently in use. This confirms that the unobligated balance of $119.58 will not be used.
