PEAK VISTA COMMUNITY HEALTH CENTERS
In our NAP grant, Peak Vista proposed a Mobile Family Care Clinic, offering a full continuum of medical and dental services with referrals for behavioral health services. The Pikes Peak region contains nearly 600,000 residents, with 22.5% living below 200% of FPL. Our target population for the mobile clinic is primarily children, of which 43% are below 200% FPL and 27.8% qualify for free and reduced lunch. This clinic focuses on two distinct populations: 1) children (0-20) and their families; and 2) the homeless. We provide services at schools to children and their families during the day and provide evening care services to the homeless populations at the local soup kitchen and other sites. Peak Vista is working with three local school districts and the local providers of services to the homeless to establish a schedule of services. The clinic is modeled after our Family Health Centers and provides access to care for children, their families, and the homeless.

This NAP project term ended February 28, 2011. In the final quarter of our NAP grant, we continued to see great consistency in scheduling of Ronald McDonald Care Mobile (RMCM). January saw 15 scheduled days and February saw 15. In the first year of the grant, RMCM scheduled stops on 151 days, on average 16.78 days per month. (In the first year of operations (March 2009 – February 2010), RMCM was on the road 9 months. The first 3 months of the grant were dedicated to the start up process.) In the second year of the grant (March 2010 – February 2011), RMCM scheduled stops on 201 days, on average 16.75 days per month. We successfully met our goal to provide an average of 16 days per month of service on RMCM.
We also successfully met our goals to provide 3,225 new patients with access to care and to provide 15,215 patient visits through this grant. By our fourth quarterly report, we were able to provide 3,618 patients with 7,029 patient visits. By the end of our NAP grant, we have seen 8,847 new patients for 50,074 patient visits both on and off RMCM. These numbers include 2,982 uninsured patients. These visits include 44,068 medical visits, 4,148 dental visits and 1,858 behavioral health visits. By far, the greatest benefit to our patient population through this has been the increased access to medical care at Peak Vista, accounting for 88% of the patient visits.
Without ARRA NAP funding, RMCM would not have been as successful as it has been. This funding has allowed Peak Vista to greatly increase access to care for many patients in many parts of our region, as evidenced by one patient’s personal story:

This quarter our NAP grant supported the following jobs: 7.3 FTE medical, 2.18 FTE dental services, 1.3 FTE pharmacy, 1.0 FTE case manager, 1.0 FTE outreach worker, 1.50 FTE other enabling services, 1.0 FTE administrative and 1.0 FTE facilities staff. All of these positions provide direct support to the patients accessing care through our Mobile Family Care Mobile. |
