With the award of 330 ARRA/HRSA funds in March of 2009, Community Medical Services (CMS) was able to re-open dental and medical services in Uniontown, PA. Community Medical Services worked with Cornerstone Care, an established FQHC in southwestern Pennsylvania, to facilitate this worthwhile endeavor. The dental practice was opened on June 9, 2009. The medical practice in Uniontown re-opened on September 1, 2009. Two part-time Family Practice Physicians, one part-time Internist and two medical assistants were hired to see patients in the Uniontown office. Patient activities at the medical snd dental practices reflect the following: 990 new patients seen since grant inception; 15.96% of these patients were un-insured.
As directed during HRSA's site visit additional TA was secured to assist in transferring this grant to Cornerstone Care, Inc. This grant (H8ACS11363) is now closed and an organizational change has been made. The grant was transferred to Cornerstone Care, effective May 1, 2010, grant number H8ACS17259. |
