The purpose of the Beacon Community Cooperative Agreement Program is to provide funding to communities to build and strengthen their Health Information Technology (Health IT)infrastructure and exchange capabilities to demonstrate the vision of the future where hospitals, clinicians and patients are meaningful users of Health IT, and together the community achieves measurable improvements in healthcare quality, safety, efficiency, and population health.

| Quarterly Activities/Project Description |
1. Data sharing agreements for the Greater New Orleans Health Information Exchange (GNOHIE) are in place with 16 organizations (13 primary care practices, 1 behavioral health provider, and 2 hospitals). Interface development and Clinical Data Repository backfill work with 24 primary care practice sites and 1 hospital has been completed.
2. 19 primary care practice sites and 1 hospital are live on the GNOHIE ED/Inpatient Notification system. An additional 10 primary care practice sites and 1 hospital will go live during Q2 2013.
3. The GNOHIE patient consent collection process commenced on July 5, 2012 and 19 primary care practice sites are currently collecting consent. An additional 10 primary care practice sites and 1 hospital will begin collecting consent in Q2 2013.
4. A telemedicine pilot involving 1 hospital (providing telemedicine appointments for 11 specialties) and 5 primary care practices was finalized in September 2012 and commenced in November 2012.
5. The GNOHIE Electronic Specialty Care Referral system is being developed. The system will be piloted in Q2 2013 and will be expanded to 29 primary care practice sites and 2 hospitals during Q3 2013.
6. CCBC engaged the Primary Care Development Corporation (PCDC), which throughout 2012 and Q1 2013 provided hands-on practice coaching and implemented a learning collaborative with 16 primary care practices.
7. The mobile health for diabetes campaign (txt4health) was launched on 1/31/12. As of April 2013, 1728 individuals have enrolled.
8. Alignment with state HIT initiatives, including the Louisiana Health Information Exchange and REC program, continues to be integrated into local efforts.
9. Data reporting workshops and technical assistance are provided to the CCBC partner primary care practices on an ongoing basis.
