Purpose: Minor blunt head trauma in children is common. Although important traumatic brain injuries are infrequent, clinicians obtain cranial computed tomography (CT) in up to 50% of children with minor blunt head trauma, exposing them to radiation with its potential to induce malignancies. We recently validated blunt head trauma prediction rules, providing strong evidence on which to recommend which children do not require CT. There is a need to study whether implementation of these rules using computerized clinical decision support (CDS) can decrease unnecessary use of CTs. Objectives: 1) to develop and pilot test an integrated electronic health record (EHR) blunt head trauma data collection template and computer-based clinical decision support system to implement the Pediatric Emergency Care Applied Research Network traumatic blunt injury prediction rules for children and 2) to assess whether implementing the prediction rules via computerized CDS decreases the number of (unnecessary) cranial CTs in children with minor blunt head trauma at very low risk of traumatic brain injuries. Methodology: In order to meet objective one, we will: a) conduct focus groups of clinicians and perform emergency department workflow evaluations, b) develop the EHR template, c) develop the CDS, and d) refine and pilot test the integrated system. In order to meet objective two, we will conduct a 7-center clinical trial that assesses CT use prior to and after the implementation of the integrated electronic health record-clinical decision support system. Expected Outcomes: We will develop and implement an integrated EHR template-decision support system. Thereafter, we will complete the clinical trial. Present Quarter Activities: We continue data collection in the clinical trial. We successfully implemented the CDS at all pediatric emergency medicine sites and will implement the CDS at the general emergency medicine sites in the next two months. |
