UNIVERSITY OF WASHINGTON
Low back pain is one of the most important causes of functional limitation and disability, an Institute of Medicine priority condition, and it remains a particularly important problem for the elderly, an AHRQ priority population. While there are numerous guidelines regarding the use of diagnostic testing and treatments for the general low back pain population, there is a paucity of evidence-based guidelines for the elderly. This is more than a mere academic concern. The annual direct medical costs of low back pain were $86 billion in 2005 and by 2010 are likely to rise over $100 billion. Despite the high prevalence and cost of low back pain, important questions remain unanswered regarding the comparative effectiveness of most commonly used diagnostic tests and treatments. Our primary goal is to establish a large, community-based registry of elderly patients with low back pain. This registry will provide a foundation for rigorous prospective studies including large, practical, randomized controlled trials (RCTs). Alongside the registry, we propose to enroll patients into two studies: 1) an RCT of elderly patients with spinal stenosis to evaluate the effectiveness, cost and safety of combined epidural steroid injections and local anesthetic compared with local anesthetic injections alone, and 2) a prospective observational cohort study of early advanced imaging (MRI and CT) compared to no advanced imaging in elderly patients with new episodes of low back pain. These studies will serve as demonstration projects for the value of the registry for comparative effectiveness research. The registry will provide the infrastructure for future studies to evaluate back pain care at multiple stages of development, from the early market penetration of relatively immature technologies (e.g. artificial discs, genomics) to widely disseminated mature interventions (e.g. MRI, physical therapy, spinal fusion). The overall goal of this project is to establish a sustainable and rich registry to evaluate prospectively the effectiveness, safety, and cost-effectiveness of interventions for patients over age 65 with low back pain.

The LESS RCT: The RCT is actively recruiting subjects at sixteen sites: Brigham and Women's Hospital, Harvard Vanguard Medical Associates, Massachusetts General Hospital and St. Elizabeth's Medical Center in Boston, Henry Ford Health System with West Bloomfield Center in Detroit, Kaiser Permanente Northern California at Redwood City, San Jose, Roseville and Martinez, the University of Colorado in Denver, Mayo Clinic in Rochester, OHSU in Portland, Stanford University in Redwood City, Virginia iSpine Physicians in Richmond and the VA Medical Center in Dallas. Due to very low recruitment we closed the site at Harborview Medical Center in Seattle. To date, we have screened 1936 patients. Of those screened, 405 (21%) were eligible and 362/405 (89%) were randomized: 117 in Detroit, 61 in Northern California, 117 in Boston, 21 in Denver, 4 in Seattle, 11 in Rochester, 5 in Portland, 14 at Stanford, 2 in Richmond and 10 in Dallas. The follow-up rate at 6-weeks, which is our primary endpoint, is 98%. We are on track to meet our recruitment goal and project that we will reach the target enrollment of 400 by late spring 2013. The BOLD Registry: We reached the recruitment goal of 5000 subjects. We closed recruitment at two sites: Kaiser Permanente Northern California and Harvard Vanguard Medical Associates. We continue recruiting subjects at Henry Ford Health System in order to enroll our target number of minority subjects. We are planning to close recruitment at Henry Ford at the end of March.To date, we have screened 13204 patients and enrolled 5244 subjects. The follow-up rate is 89% at 3-months and 85% at 12 months. We are exploring the possibility of additional funding in order to complete the 12 month follow-ups for all subjects at Henry Ford and to extend the follow-ups for all registry subjects to five years. On March 13, we submitted a grant application to PCORI "Comparing Engagement Techniques for Incorporating Patient Input in Research Prioritization". |

The Research Coordinators primary responsibility is the protection of human subjects while carrying forward the research goals of a project. |
