Equipment requested will expand training capabilities. The Patient-Centered Medical Home (PCMH) model of integrated team-based care fosters partnerships between patients and their personal physicians. We seek to support interprofessional training of faculty, residents, students, and staff to enhance the educational infrastructure of a PCMH in the Department of Family and Community Medicine and three community clinics: Southside United Health and Wellness Center and Centro Clinico (both urban underserved clinics), and Foothills Primary Care (rural clinic 50 miles away). These sites provide a rich interprofessional learning environment; faculty and learners participate in clinical care across multiple sites.
This equipment will facilitate: 1) provision of effective patient-centered care, 2) education on attitudes, knowledge and skills for an effective PCMH, 3) increased potential for learners to choose and remain in Family Medicine and create PCMHs in underserved communities, and 4) strategies for better integrated care for underserved patient groups.
