Project Title Modulation of Cystatins and Cathepsins in HIV-1 Neuropathogenesis

Quarterly Activities/Project Description Our laboratory has shown increased secretion and neurotoxicity of cathepsin B from in vitro HIV-infected monocyte-derived macrophages, and increased expression in post-mortem brain tissue with HIV encephalitis and HAD. We hypothesized that cystatin B and cathepsin B could represent potential biomarkers for disease progression. Monocytes, plasma, and CSF from 98 HIV seropositive Hispanic women cohort were selected for this study. These included 75 patient visits stratified by normal (22), asymptomatic neurocognitive impairment (ANI) (23), and HAD (28). Thirty-two patients at two consecutive visits were stratified into progressors and non-progressors. Samples were evaluated for cystatins B and C, and cathepsin B expression and activity. Data was analyzed using Kruskal-Wallis one-way analysis of variance. Increased cathepsin B and cystatins B and C were found in plasma of HIV seropositive women. Higher expression of cathepsin B and cystatin B were found in monocytes from women with HAD (p<0.05). Significant decrease in cathepsin B activity in CSF was found in demented compared to asymptomatic women and in monocytes from progressors. These results demonstrate that dysregulation of cystatin B-cathepsin B system is operative in HIV associated neurocognitive disorders (HAND) and suggest that expression of cystatin B and cathepsin B in monocytes could be potential biomarker for disease progression. Overall Significance of this project. Our results are important for diagnosis and therapy of HAND and have been published in the J Neurovirology (Rivera et al, 2011), PlosONE (Rodriguez-Franco et al, 2012), and the patient-related research will be submitted to AIDS.
