Drug overdose is the single greatest cause of mortality among injection drug users (IDUs) in the U.S. Fatal heroin overdoses cause more than half of all deaths among IDUs, which far exceeds the proportion of deaths due to AIDS or other causes. Administration of heroin or other opiods via intravenous route not only increases the risk for HIV and other blood-borne pathogens, but also significantly increases the risk a of potentially fatal overdose. Therefore, IDUs are a population at elevated risk for both HIV infection and fatal drug overdose. A heroin overdose can be safely reversed with the administration of naloxone (brand name Narcan) – a legal, nonscheduled opioid antagonist. Naloxone is currently prescribed and distributed by overdose prevention programs (OPP) in 15 states. Despite the common-sense notion that these programs “work,” scientifically designed evaluations of OPPs are almost non-existent, and outcomes are still poorly defined. The proposed study is designed to investigate changes in overdose response and other drug-related health behaviors among IDUs participating in two OPPs in Los Angeles. The study will address the following three specific aims: describe overdose response behavior among IDUs; examine whether participating in overdose prevention training or overdose response (i.e. performing a “rescue”) is associated with other drug-related health behaviors; and investigate programmatic, social, and psychological barriers to enrolling IDUs into OPPs. The proposed study will use qualitative and quantitative methods to study samples of IDUs recruited from two community-based programs in Los Angeles that conduct overdose prevention training – Homeless Health Care Los Angeles (HHCLA) and Common Ground (CG). Quantitative baseline and “incident report” data will be collected as part of an existing protocol started in January, 2009, to document behaviors in response to witnessed overdoses and circumstances precipitating requests for refills of naloxone. In-depth interviews with 200 IDUs (100 trained and 100 untrained) will explore rationales for and barriers to enrolling in overdose prevention training, the effects of training participation, differences between training programs, and the effects of participating in an overdose rescue.

Description of Jobs Created PRINCIPAL INVESTIGATOR (Stephen E. Lankenau, PhD; Assistant Professor of Pediatrics and Preventive Medicine, Keck School of Medicine of the University of Southern California. He is budgeted at 2.4 person months for Cal Year 1 and Year 2). Job description: Dr. Lankenau will be responsible for overseeing and supervising all aspects of the research activities including subject recruitment, data collection, data management, data analysis, and dissemination. He will provide oversight of all CHLA project staff and coordinate the activities of the consultants and program sites to ensure that all aspects of the study design and project activities are developed and implemented as proposed. He will also perform administrative responsibilities, make executive decisions regarding the administration of the grant, and oversee the project budget. Finally, he will prepare written reports and make oral presentations on findings and policy relevance to professional audiences and for publication in peer-review journals. DATA ANALYST (Karla Wagner, ABD, budgeted at 6.0 person months in Cal Year 2). Job description: Ms. Wagner will contribute to the conceptualization, design, and implementation of the proposed research, particularly regarding data analysis during the final six months of the study. She will meet with the Principal Investigator, Ethnographer, and Consultants to discuss findings and analytical approaches. She will be responsible for conducting both quantitative and qualitative analyses as outlined in the proposal, and will actively participate in the reporting of study findings. Ms. Wagner will complete her NIDA-funded R36 award before being hired onto the study. ETHNOGRAPHER (Meghan Treese, Childrens Hospital Los Angeles, budgeted at 12.0 person months for Cal. Year 1 and 6.0 person months for Cal. Year 2). Job description: Ms. Treese will be responsible for conducting qualitative data collection at the program sites, including field observations and in-depth interviews with trained and untrained participants. Ms. Treese will meet regularly with the Principal Investigator to provide updates on the progress of the qualitative activities, and to communicate any problems that arise during the course of research. Ms. Treese has conducted qualitative data collection on three previous NIDA-funded studies over the past four years. Her job skills and experience that relate specifically to the proposed study include: screening and recruiting injection drug user; conducting qualitative interviews with active drug users; conducting participant observation in public and private settings; and recording field observations into field notes. PROJECT COORDINATOR: (Jennifer Jackson Bloom, Childrens Hospital Los Angeles, budgeted at 1.2 person months in Cal. Year 1 and 1.2 person months in Cal. Year 2). Job description: Ms. Bloom will assist with the development of research design tools and strategies, including the development of the study protocols. She will program the interview protocols, and oversee quantitative and qualitative data management. She will perform quality control checks, and ensure adherence to confidentiality and human subject procedures. Ms. Bloom has performed similar roles on several NIDA-funded studies. TRANSCRIBER: (Donna Luebbe, Childrens Hospital Los Angeles, budgeted at 1.2 person months in Cal. Year 1 and 1.2 person months in Cal. Year 2). Job description: Ms. Luebbe will be responsible for transcribing all indepth interviews with trained and untrained participants. She will work closely with the Ethnographer and Project Coordinator to ensure that the interviews are accurately transcribed. Specifically, she will access digitally recorded files from a research-designated, password-protected server. She will transcribe and then save each completed transcript to the designated folder for the Ethnographer and Project Coordinator.
