The purpose of this four-year study, 'School Worksite Weight Gain Prevention Intervention Study', is to determine the effectiveness of an ecological worksite intervention compared to a self-directed, print-materials only control group at reducing weight gain among public high school employees in Massachusetts. The ecological intervention is a comprehensive program targeting the three levels suggested by ecological frameworks for health behavior change: (1) the organizational (school) level, (2) the interpersonal level, and (3) the individual level. A social marketing campaign will integrate all program activities using common logos, themes and messages. Evaluation of the proposed intervention compared to the print-materials only control group will be by a randomized controlled trial (RCT) in which the unit of randomization and intervention is the school worksite and the unit of evaluation and inference is the employee. Twelve schools will be stratified into two strata large urban or small urban schools and community socioeconomic status (based on census tertiles). Within each stratum, schools will be randomized to either the ecological intervention or print materials only control condition. A total of 540 teachers and staff from 12 schools will be recruited and retained, 270 from intervention and 270 from control sites. Assessments will be completed at three time points: baseline, 12 months and 24 months. The primary aim of this study is to determine the effectiveness of the intervention in preventing employee weight gain, as measured by change in BMI and waist circumference, over the 24- month intervention period. Secondary aims include determining the effects of the intervention on physical activity, saturated fat intake and consumption of fruits and vegetables among employees, as well determining the effects on absenteeism and presenteeism. Tertiary aims are to determine the effects of the intervention on mediating and moderating factors, including perceived organizational commitment to employee health, perceived co-worker normative influences for healthy eating and physical activity, social support for healthy behaviors, and self-efficacy. The proposed study builds off of the study teams previous studies and experiences in designing, implementing and evaluating ecological worksite interventions and in conducting large-scale randomized controlled trials in the high school setting. There are three phases to the investigation: Phase I: Recruitment of schools and intervention and instrumentation refinement; Phase II: Intervention implementation and evaluation; Phase III: Analysis and dissemination. PUBLIC HEALTH RELEVANCE: The proposed site-level randomized trial will test the effectiveness of a worksite intervention based on an ecological framework at preventing weight gain among high school employees in Massachusetts. If effective, the intervention is designed to be generalizable to high school settings across the country and has the potential to reduce the morbidity, mortality and economic costs of overweight and obesity in this important population.

We are in Phase II (Intervention Implementation and Evaluation). These tasks have been completed: 1) At each school in the enhanced intervention condition, 1 or 2 staff members who serve as the Step Ahead coach are paid by the study to devote a few hours each week, beyond their usual responsibilities at the high school, to plan, implement and promote intervention activities. School nurses, physical education teachers, librarians, and classroom teachers serve in these positions. 2) Employee Advisory Groups have met at least twice at each of the schools randomized to the enhanced intervention condition. The purpose of the EAG is to brainstorm ideas as to how to improve the environment for healthy eating and physical activity, and to assist the coach and study staff with program planning, implementation and promotion. 3) Employees at the 6 enhanced schools continued to participate in walking challenges, using high-quality pedometers provided in the fall. Prizes were awarded, including items for each member of the winning teams, for improvement in number of steps, and for the person at each school who walked the most steps. Additional prizes were given for participation. A 5K run/walk was held at 1 school. 4) All of the enhanced intervention schools are working on providing or improving staff access to fitness centers and showers, and providing healthy eating options for staff. The study goal is to have such changes reflected in written policy, to have policy followed or maintained. 5) Exercise classes are being offered at all enhanced schools. Classes include zumba, kickboxing, yoga, pilates, walking and strength training combined, and mixed activity. Weekly basketball games are held at 2 of the enhanced schools. 6) Strength training classes have been provided. Each employee who attends a class receives a stretch band to keep and learns a short routine exercising all major muscle groups. The unspent funds will not be utilized, thus will be relinquished. |
