OREGON COMMUNITY HEALTH INFORMATION NETWORK, INC
The OCHIN Transforming Care Quality (TCQ) project seeks to positively impact the health status of patients with diabetes who are served by five different community health center organizations located in Oregon and California.
This project will support OCHIN quality improvement (QI) staff to work directly with site care teams in primary care clinic sites to use the data captured and reported from their common electronic health record and quality improvement systems to identify and implement work flow changes needed to improve care for patients with diabetes.
There are three specific measure goals of this project, they are: 1) percent of patients diagnosed with diabetes with HbA1c test performed in the last 6 months is 75% or greater; 2) percent of adult patients diagnosed with diabetes and screened for depression within the last 12 months is 25% or greater; and 3) the percent of patients diagnosed with Diabetes where the last HbA1c test resulted in a reading of <=8 in the last 12 months, the goal is 70% or greater.

In this final quarter, the emphasis continued on spreading indentified improvements from the project pilot sites within their organizations. The majority of the sites met their spread goal by the close of this grant and spread will continue beyond this award.
During this final year of the project, core team meetings continued at the clinic sites and all participating clinic specialists continued to share best practices and work towards our final project goal of bringing HbA1c control above 70%. While we experienced considerable success in increasing the HbA1c control measure near 70% (currently 68.4 for TCQ participants), it is important to note that this measure is a true clinical
outcome rather than a process measure, which is easier to affect as the provider is in more control of a process measure. The HbA1c control measure requires that responsibility also be placed on the patient and includes forces, which are in large part, outside of the control of the clinic within the timeline allowed by this grant. Though we did not reach our goal of 70% for the control measure we have discussed this with our Project Officer and agree that this would not hinder our ability to close down this grant. |
