Quarterly Activities/Project Description Activity Narrative: The HPRP program has two components: Homelessness Prevention Program and Rapid Re-Housing for Singles and families. The Homelessness Prevention Program is a partnership between the Greater Cincinnati United Way, Hamilton County, and the City of Cincinnati. Strategies to End Homelessness, Inc. fka Cincinnati/Hamilton County Continuum of Care for the Homeless, Inc. (CoC) developed this program and began implementation on September 17, 2009. Provider agencies, identified jointly by the United Way and the CoC, provide direct services to individuals and families requiring prevention services. United Way dollars will be used for case work services; HPRP dollars of the City will be used for Financial Assistance. No less than 1,600 cases will be served, over the three-year period, with these funds. A total of 139 individuals (60 households) received services this quarter. A total of 150 individuals exited the prevention program in the 3rd quarter of 2012 and 6 individuals left the rapid re-housing program in the 3rd quarter of 2012. This program is complete and there will be no more activity. The Rapid Re-Housing program will provide rapid housing to homeless single individuals as well as families within the shelter system. Individuals and families eligible for participation in this program will be current residents of one of the emergency shelters providing shelter for singles individuals or families as identified by the CoC, Inc. The provider agency for HPRP-RRH for singles and families has been identified as the Freestore Foodbank, Inc. No less than 150 single individuals will be served, over the three-year period, with these funds and no less than 100 families will be served the remainder of the grant period.

Description of Jobs Created HPRP FTE Narrative 3RD. Quarter 2012 Work performed on the City?s Homeless Prevention and Rapid Re-Housing (HPRP) Program began on September 17, 2009. During the 3rd quarter of 2012, the HPRP program created 3.12 FTE?s. The following professional positions were created: Benefits Specialist (.34 FTE), HPRP Coordinator (.10 FTE),Assistant HPRP Coordinator (.10 FTE), Six Case Managers (1.88 FTE), Two Housing Specialists (.30 FTE), HPRP Bookkeeper/Finance Coordinator (.16 FTE), Finance Assistant (.11 FTE), Admin. Assistant (.01 FTE), Finance Director (.11) During the 3rd quarter of 2012, the HPRP program retained .05 FTE?s. The following positions were retained: RRH Coordinator (.03 FTE), STEH Executive Director/HPRP Coordinator Supervisor (.02). A total of 10.56 FTE?s are reported for this quarter for HPRP.
