COUNTY OF VOLUSIA
Volusia County through its Community Assistance Division will implement the HPRP and directly provides services to the program participants. HPRP staff will provide program training and technical assistance to the Continuum of Care (CoC) partner agencies and other agencies that will serve as referral sources to HPRP. Individual and families who are "at risk" of becoming homeless and need temporary assistance will be identified through Community Assistance staff and referred to the HPRP case manager for a consultation, assistance focused on housing stability, and linkages to community resources and mainstream benefits. To assist individuals and families who are experiencing homelessness (residing in emergency or transitional shelters or on the street) and need assistance in order to obtain housing and retain it, a referral process will be implemented through Memorandums of Understanding (MOU's) with the CoC partners to the HPRP case manager. The HPRP case manager will collaborate with the referral agency's case manager to develop a re-housing plan. The plan will include the types of assistance needed to transition the participants to independence and permanent housing stabilization.

01/12/12 - The County of Volusia and Awarded Agency's, Rapid Re-housing, are working together to ensure the clients turn in all appropriate paperwork and to ensure they are successful in completing HPRP. The agency case managers monitor the clients and make sure the clients are working, looking for work, etc. and saving (trying to save) funds so that when HPRP funds have been exhausted, the client is successful in maintaining financial stability and can afford day-to-day and month-to-month living expenses. Those who are homeless (Rapid Re-housing).Approximately 35% of HPRP funds will be allocated to provide the following assistance and housing re-location and stabilization services: Case management, Short/medium term rental assistance, Security/ utility deposits, Utility payments, Moving costs assistance, Motel/hotel vouchers. Rapid re-housing will assist individuals and families who are experiencing homelessness (residing in emergency or transitional shelters or on the street) and need temporary assistance in order to obtain and retain housing. 12/19/11 - Reallocation of funds to the case management budget to provide services through the end of the grant term - Transferred Case management funds from the following agencies to Rapid Re-Housing Program Funds: Family Renew Community - $1,495 and Halifax Urban Ministries $8,928 = $10,423. Transferred Case management funds from Halifax Urban Ministries - $2,543 to Haven Recovery Center Case Management funds. |
