Increased Demand for Services Purpose: Place Patient navigators in five Sub-Regional Clinics operated by the Yukon-Kuskokwim Health Corporation. Goals: To provide individualized assistance for patients during registration, translation when needed during clinical visits, and assistance registering for Medicaid/Medicare/SCHIP services. This consists of in-clinic and outreach to the surrounding communities. 2) Work with registration clerks to ensure that patients eligible for Medicaid / Medicare are registered appropriately 3) As times allows perform a narrow definition of case management as defined below a) High risk patients as identified on monthly reports will have an appointment set up with the provider. Working with the provider the chart will be reviewed for appropriate labs etc. These to be scheduled for the patient b) Assist patients with referrals, travel etc.
