survival at 5 year is very low,there is a need for a better understanding of the
pathogenesis of the disease.
These studies will permit a
prospective analysis of the
histological and molecular
effects of dietary components
and bacterial carcinogens
during the early and late stages of carcinogenesis in a primate model.

H. pylori, carcinogens, and protectant isoflavones play critical roles in gastric carcinogenesis.
Twenty-three macaques were divided into 4 groups: controls (C); oral carcinogen ethyl-nitro-nitrosoguanidine (ENNG) (E); infection with virulent H. pylori strain (H); and ENNG plus H. pylori inoculation (EH). Gastroscopies with biopsies were performed at regular intervals for 6 years.
Gastric cancer appeared in three EH macaques but not in any E, H or C animals. In addition, all E and EH macaques, but no C or H macaques, developed gastric angiodysplasia well visualized by computed virtual chromoendoscopy. The new low isoflavone diet was started and is well accepted by the animals.
We have initiated a modification of the diet of the macaques to a low isoflavone diet that enhances the carcinogenic effect of H. pylori and the carcinogen. The new IACUC protocol describing implementation of the new diet was approved. A low isoflavone diet was prepared by Harlan and a sample was tried on the macaques to habituate the animals to the new diet. Complete switch to the new diet will be done when the animals have been completely adapted to the diet.
In addition, we collaborate with Dr. Moss' group to perform Laser Capture Microdissection on archival specimens. These studies will permit to discover the genes that are overexpressed in the mucosal and submucosal regions of the stomach of animals that develop cancer in subsequent years. In turn, this discovery could lead to tests to predict cancer and to prevention of cancer.
Conclusions: H. pylori-induced inflammation plus ENNG can cause experimental gastric cancer in a non-human primate model. In the same model, ENNG exposure promotes angiogenesis and induces phenotypic changes similar to those observed in humans. The present studies will permit molecular analysis of these effects and may help in the development of new methods to predict and prevent gastric cancer.
