Quarterly Activities/Project Description January, 2011: Continued to refine clinic workflow dealing specifically with ER and hospital follow up walk ins. Workflow meetings included staff from Clinical Support Services, front desk, internal medicine nursing staff, and clinic manager. Durable Medical Equipment (DME) workflow reviewed and redesigned. Was decided that we will report total number of outstanding / unsigned DME requests at the general provider staff meeting each month. Met with Texas Medical Foundation (TMF)representatives and local hospital, to discuss Care Transitions project. Hospital will reinforce use of discharge planning / communication tool, which provides direct fax number to Clinical Support Services. February, 2011: Continue to work with hospital and ER follow up clients. Nursing staff attended diabetic self management education training workshop. Designed new documentation template for asthma, reflecting Meaningful Use requirements. Staff attended training sessions for new hospital document retrieval system (PIE). Su Clinica is the largest clinic site accessing these hospital records, so the hospital wants our feedback regarding functionality of the new system. March, 2011: Job descriptions evaluated and updated, reflecting current IDS activities. LVN attended Children with Special Healthcare Needs webinar, topic of transitioning from pediatric to adult.
