SALUD INTEGRAL EN LA MONTANA INC


The Increased Demand for Services (IDS) project proposed includes two strategies to start on July 2009: to strengthen urgent care services during evening hours and expanding ambulatory care/outreach services. The goals of the proposed project are: 1) to strengthen the urgent care services during evening hours, seven days a week, at the health center site located in Naranjito adding a part time physician (.5 FTE) to offer services from 4:00 p.m. to 8:00 p.m. during week days; 2) to expand the existing X-rays services Monday-Friday until 9:00 p.m. and Saturdays from 8:00 a.m. to 4:30 p.m. contracting a part time (.63 FTE) Radiology Technician; 3) increase of nutritional, health education and clinical outreach services to increase access to health care services and primary prevention within its service delivery area. These strategies will allow the organization to expand access to health care services to people that may be displaced from jobs and to identify new uninsured and under insured persons in the communities served.

Quarterly Activities/Project Description As a result of ARRA funding, from April thru June 2011, SIM was able to provide new services to 265 new unduplicated patients (including 7 seasonal farm workers) and 1,060 current health center patients. The main services provided to new unduplicated patients were urgent care services (233 patients or 88%), while the new services provided to current SIM patients (added on SFPPR Page 2) attained a distribution of 59% nutrition services (597 patients), 15% health education services (149 patients), 26% case management services (260 patients) and 5% mental health services (54 patients). The primary diagnosis distribution among patients reported includes: 20% diabetes, 19% hypertension, 3% cardio-vascular disease, 3% asthma/COPD and 20% dyslipidemia. The most significant contributing factor to the reported results was the increased capability of the provision of nutritional and enabling services at all Health Center sites during this period as a result of the recruited clinical personnel with ARRA funding. The Enabling Services clinical staff recruited with ARRA funding were able to provide 114 on-site and outreach community/patient-oriented healthcare empowerment promotion activities, which included health booths and educational sessions with multiple topics. The overall assistance to the activities was 2,900 participants.
