For nearly 80 years, SPHC has provided services to meet the health and wellness needs of the community of St. Paul. Recently, however, meeting behavioral health needs has become a significant challenge. Due to the remote location and lack of support staff, APIA has been unable to fill the position of Behavioral Health Clinician (BHC) at SPHC since May 2008. Without this position, SPHC cannot provide behavioral health services in St. Paul. At present, the only behavioral health services available to St. Paul residents are counseling, follow up, and minimal case management provided telephonically and telepsychiatry.Goals and Objectives: APIA proposes to expand behavioral health services on St. Paul Island to address issues of Access, Utilization, Risk Management, and Financial Viability. Specifically, APIA proposes to accomplish the following four goals: ?Establish a resilient, locally relevant behavioral health program.
? Increase cultural acceptance of behavioral health services.
? Improve the accuracy and timeliness of recordkeeping.
? Improve the financial sustainability of the behavioral health program. As shown in Criterion 1, the Behavioral Health Program has historically suffered from significant vulnerabilities. Due to the high level of training necessary to hold this position, APIA has never hired a resident of the local community. In order to make the program more resilient, it must become an integrated part of an island-wide system of care, based on local priorities and staffed by local residents. APIA therefore proposes three objectives for this goal.
? Recruit and hire a Behavioral Health Aide, with a local resident hiring preference.
? Develop a Behavioral Health Crisis Intervention System with other agencies on island.
? Establish a full range of therapeutic and counseling opportunities on island. Historical and cultural factors have combined to create barriers to acceptance of, and therefore access to, behavioral health services on St. Paul Island. APIA proposes three objectives to accomplish this goal.
? Improve confidentiality of behavioral health services.
? Implement school-based prevention, education, and outreach program.
? Implement integrated primary care and behavioral health protocols. With the entire behavioral health program relying on the services of a transplanted provider, turnover has proven to be a significant issue. APIA will impose a requirement that at least 90% of client records are updated within 72 hours of clinical appointment with 100% accuracy. New protocols are implemented through the following objectives:
? The Behavioral Health Program Policies and Procedures Manual will include updated documentation parameters, including additional clarification on required information, referral protocols, and timing expectations.
? The Behavioral Health Program will publish an annual schedule and protocols for quarterly on-site chart reviews by the APIA Behavioral Health Director.

Throughout this quarter, the Behavioral Health Aide (BHA) continued education through a Health Insurance Portability and Accountability Act training. The BHA continues to be available to provide community services which include intakes for the substance abuse and mental health program, outreach intervention within the community, assistance with paperwork, food bank requests, Head Start social skills and work with the youth group.
Opened the new year with volunteering at Elder?s New Years Ball which was filled with record music, dancing and laughter. The evening was all in all a great social event for the elders to socialize with not only each other but the youth in the community as well.
Aided in community education through the creation of media such as fliers, brochures, slideshows and public service announcements. All was completed to publicize the Electronic Health Record Go Live event taking place at the health center and inform the community with pertinent information.
Collaboration with the Tribal Government of St. Paul continues to positively grow through taking part in activities such as planning of the International Fur Seal Treaty Centennial; Celebrating Conservation and Culture, Pribilof Islands, Alaska which will prove to be a great cultural gathering with history review over the seals and their effect on our culture cultural dance and crafts as well as traditional food. The event will provide for the people of the community to be immersed in their cultural history and traditions. Also, being a member of the Constitution Working Group and through taking part in the Tribal Health Department meetings such as the Sexual Assault Response Team and the Strategic Planning Advisory Board. All to work towards providing the island residents with a cohesive service. |

This is an entry level village based counseling position that provides village residents (individuals and families) with basic education about mental health, substance abuse, tobacco, and addiction issues. With direct assistance and oversight from the Clinical Consultant, the BHA will screen and/or assess individuals for mental health and substance abuse problems. Interacts and coordinates professional activities with other village-based providers, helping to provide early intervention, case management, aftercare and follow-up for individuals and families impacted. Pursue education through on-the-job training, specific training opportunities and through the University Rural Human Services Certificate Program. |
