Quarterly Activities/Project Description CHCI has made significant improvement in Hypertension control (HTN) as a direct result of funds obtained through this grant. Currently our HTN agency rate is 61% controlled as compared to 54% at our start. This has been achieved by development of the dashboard, team Panel meetings, Pharm D e-consults, Home blood pressure monitoring, patient engagement in establishing Self Management Goals, and Grand Round lectures.
