KOKUA KALIHI VALLEY COMPREHENSIVE FAMILY SERVICES
The purpose of this award is to expand access to needed primary health care services specifically by expanding services at existing health center sites in an underserved area or for an underserved population. The IDS project that KKV will be implementing involves expanding existing services with new hires, extending hours of primary medical care operations at satellite service sites, and retaining jobs that are connected to community-based preventive health care programs.
With this award, KKV committed to hire a new Licensed Clinical Social Worker to deal with the increasing needs within the community that come from job lay-offs, lack of work opportunities, and concurrent related issues such as increased substance abuse, domestic violence, gang violence, depression, anxiety, chronic stress disorders and other behavioral health problems. 2 new clinical support staff will be added to allow a new KKV physician to expand hours of primary care services at two separate KKV satellite sites -- at its Elder Services Center and at the Kuhio Park Terrace public housing complex (the largest in the State of Hawaii). Expanded clinical service hours at these sites will accommodate the growing demand for these types of services in the areas where these satellite sites are located, and will circumvent the problem of limited space for patients at KKV's main health center.
In addition to new hiring, KKV also committed to retain a .2 FTE LPN and 2.25 FTE of case management and other paraprofessional staff. These staffs have been providing culturally and linguistically competent outreach, case management and intervention activities with community residents who have substance abuse and other behavioral health related problems and eligibility services.

The IDS funding gives the opportunity to Kokua Kalihi Valley to retained 4.20 new FTE consist of a 0.4FTE License Clinical Social Worker (LSCW), 1.0FTE a Social worker (MSW), 1.0FTE a support staff for medical department (Medical Assistant) and 1.0FTE a case manager and 0.8 FTE of Behavioral Health case manager position. By retaining the eligibility staff and case manager position makes possible for KKV to continue to provide culturally and linguistically competent enabling services including outreach, case management, medical interpretation and assistant for enrollment in government health plan like Medicaid and other government benefit programs. In addition, KKV hours of operation at both satellite clinics has also expanding to 48 hours per month.
The funding has also increased the efficiency of the current operations at both satellite sites and allows KKV to provide more services related to the community based preventive health care programs such as chronic disease prevention and management, care giver support program and comprehensive memory clinic at both satellite clinics to low income community in those areas. As the only Community Health Center in the area, expanding the clinic hours of operation has proven to accommodate the growing demand for the services and easy access to primary care services by the community who lived in the areas. The expansion of services at both sites has also circumvent the problem of limited space for patient at KKV's main health center and decrease waiting time for appointments, open up many appointments for walk in and new patients, and provide reassurance to those unable to pay for their health care that they may be seen.
