CIALES PRIMARY HEALTH CARE SERVICES, INC.
Ciales Primary Health Services is a non-for-profit community-based organization founded in 1986 that operates a Section 330 community health center. This CIP proposal addresses the needs of the municipality of Ciales for better health services to manage acute conditions and minor trauma. The urgency/emergency health care area (UE) was constructed during the first of the years of the 1970s and no significant improvements have been made to the area. Through this proposal the center addresses the findings of the inspection/assessment done by the Puerto Rico Department of Health. The proposed CIP project has been developed to address and correct those findings. UE services are included in the current BPHC approved health center's scope of services. The implementation of this proposal will result in more effective and efficient primary services to the 20,572 population residing in the municipality of Ciales target area. This proposal will also assure the continuity of care to the center's users that will be better served in the expanded and renovated UE clinical area and will attract more patients to receive care for acute conditions locally and not at the Emergency Rooms of backup hospitals

Demolition and removal of walls, doors, floor and ceiling in the ER area. Installation of new doors with frames. Complete installation of electrical boxes and new piping for the lamp system, installation of drinkable water and sanitary piping in the utility area and minor surgery area. A/C in the minor surgery area completed. Gas system completed. All the walls in Gypsum board installed. Installation of acoustic suspension, all the areas painted, installation of wall and floor tiles in bathroom, installation of smoke detector piping, new nurse call system connected to the existing system, painting of the exterior area.
Installation of walls in gypsum board and ceiling in acoustic, installation of tile floor, installation of A/C Ducts, electrical labor and lamp installation, paint, bathroom equipment installation, pipeline for data and communication installed. 100% completed.
