PIMA COUNTY
Homeless Prevention and Rapid Re-Housing Program, aka Project Action, a collaboration between the City of Tucson and Pima County that provides case management and financial assistance in the form of rent, utility, moving, storage and motel vouchers. The program provides credit repair counseling and legal aid. The service targets clients who would be homeless with out this support or to move clients from homelessness and stay housed with the support.

As of the 12th quarter Pima County spent 100% of funds allocated. Progrm ended on June 30, 2012. All staff who worked in the program have found other jobs. As of this quarter all funds have been completly received, invoiced and drawn in IDIS |

Resource Specialist interviews client, assess eligibility, provides resources, creates and reviews case plan, enters data into Homeless Management Information System (HMIS), conducts follow-up at a minimum of every 3 months. Contract Specialist (CS) reviews requests for services which arrive via phone calls or email, conducts initial phone interview to assess initial eligibility, schedules an appointment with the Reources Specialist(RS). CS reviews HMIS client data entered by RS who issues a voucher for legal services, requests housing inspections, as needed, assures rent resonableness and cuts checks for financial assistance.
