This proposal is for a Rare Cancer Genetics Registry (RCGR). The proposal is being submitted for Challenge Area: 07: Enhancing Clinical Trials and specific Challenge Topic: Rare Disease Genetic Patient Registry: 07-OD(ORDR)-102. We will develop a registry of individuals with rare cancers who are interested in participating in clinical research. We propose to rapidly recruit a large number of individuals with rare cancers and maintain their data in a secure web-based database. Registrants will give consent to be re-contacted for participation in studies and to provide updates to their clinical information. They will give samples from which DNA will be extracted and stored in a central bank. To develop this infrastructure we will leverage the resources and scientific community of the Cancer Genetics Network (CGN). The CGN Steering Committee will guide decisions on who and what data should be in the Registry, recruit registrants from their cancer clinics, and review all requests for access to resources of the registry. We will build and maintain a web-based resource for communication about the Registry with researchers, including a query tool which can be used to characterize the registrants and samples available for studies. We will provide researchers who contact the RCGR with biostatistical support to plan and design studies using registry subjects. This project will fill an important gap in the research infrastructure, providing a resource to recruit sufficient numbers of cases needed to perform important clinical and translational research such as exploring the genetic determinants of disease and response to therapy. Such studies are often infeasible because the subject pool is small, but have the potential to reveal the etiologic mechanisms not only of specific rare cancers, but possibly of more common and complex tumors. By utilizing the existing infrastructure of the CGN, we will be able to quickly and economically develop this infrastructure within the two years of this Challenge Grant, which can subsequently be supported by research grants from NIH or other agencies. This project also serves the intent of the Challenge Grants related to stimulus of the economy, as it will require hiring personnel at the clinical sites and at the coordinating center who are not currently funded or employed. Thus, this project will be a boost to the economy while improving the research infrastructure, increasing the opportunity for new insights into the causes of and treatment of cancer. This project will fill an important gap in the research infrastructure, providing a resource to recruit sufficient numbers of cases needed to perform important clinical research including clinical trials and analyses of the genetic determinants of disease and treatment efficacy in these rare diseases. Enrollment
The RCGR recruited 612 cases, from 10 rare cancer types. Recruitment was conducted using hospital tumor registries at 5 academic health centers. Pathology reports were collected to confirm diagnoses and an accompanying DNA bank was created. At enrollment, data collected included family cancer history, co-morbidities, genetic test results, tobacco/alcohol use, and demographics. A psycho-social questionnaire was subsequently IRB approved and added to the RCGR data collection instruments. Prior to the study conclusion, centers were asked to submit diagnosis, treatment, and recurrence variables from their local registries for all RCGR registrants. Currently, the Coordinating Center has received this from 3 of the 5 centers.
Scientific Progress
Funding from the ARRA grant was for the creation of the Registry and collecting DNA. The Coordinating Center at MGH used the successful initiation of the RCGR to leverage two subsequent NIH grant proposal, both of which have received funding. Dianne M. Finkelstein, PhD was awarded an R03 in April 2012 aimed at identifying the clinical and epidemiological factors associated with elevated risk and long-term outcomes for each rare cancer. In September 2012, Dr. Finkelstein was awarded a 5-year R01 for the continuation and expansion of the RCGR registry. Enrollment will be conducted at 3 academic centers and will include collection of treatment and recurrence data, as well as DNA and tumor issue.
Website Development
The RCGR website continues to average 30 unique visitors a month during this quarter. www.rarecaregistry.org The website has a functional query tool, permitting RCGR and outside investigators to query the database. This website will be expanded for the newly funded RCGR R01.
Grant Administration
The RCGR project has met its project end date of August 31, 2012. Study staff and MGH Research office are completing the required closeout documentation as defined by the final Notice of Grant |

Our specific aims are to:
1. Populate a registry of individuals with rare cancers who consent to provide demographic, clinical, and family medical history information as well as biospecimens, and who agree to be re-contacted to participate in research and to maintain current contact and clinical information. In addition, registrants will be asked for permission to allow investigators to access their medical records and retrieve their pathologic specimens, in the event that it is required for a research study.
2. Provide a web-based data management system for secure recruitment and data collection, storage, and data management.
3. Collect, track, and store biospecimens obtained from registrants.
4. Provide a web-based resource for researchers, clinicians and the public. This will include:
a. A query tool for determining whether registrants meeting specified criteria are available for studies
b. A hub for information on web-sites and other resources for patients and researchers in rare cancer field
5. Provide biostatistical support to assist investigators in design considerations for studies using registry subjects.
