I.Specific AimsDuring the two years of funding, we will:-Establish a multi-disciplinary and cross-institutional network of health delivery systems and researchers in Western Washington State for evaluating the comparative effectiveness of cancer diagnostics in real-world settings; This partnership of organizations that will not only provide regional population-based data but also represent a variety of health care delivery systems available with a commitment to the implementation of the research findings making this truly a translational and transformative effort using real world examples and data from populations that are affected by these variations in diagnostic practices.a.Use the Seattle-Puget Sound population-based infrastructure to conduct retrospective and prospective community based comparative effectiveness research with a focus on rapid turnaround, pragmatic studies and trials. The research priorities and procedures will be informed by: systematic reviews to identify evidence gaps; discussions with key stakeholders - including payers - and community groups to establish research and policy priorities; and assessment of feasibility to perform retrospective and prospective studies.b.Using the infrastructure described above we propose to conduct two retrospective comparative effectiveness studies as "proof of principle." The first study one that our investigators and consultants agree is a high priority research topic in diagnostics, i.Comparative effectiveness of diagnostic tools used for establishing extent of disease among newly diagnosed Stage I - III breast cancer (with no clinical evidence of distant spread). We will compare the effectiveness of the use of mammography, MRI, Ultrasound, PET-CT, and new blood or tissue-based tumor markers (such as OncoType Dx) alone and in the combinations used in practice for the staging and treatment decision making in Stage I-III breast cancer.c.Using the infrastructure described above, we will develop a process for identifying priority topics from our stakeholder partners and selecting one that merits retrospective analysis. Our process for prioritizing among various diagnostic and imaging modalities and their clinical applications will involve assessment of evidence gaps, determining whether there is high variability in use, and reaching agreement among the Steering Committee that evidence is needed to improve clinical use. We will then conduct a second retrospective study focused on our stakeholder priorities.d.Design one prospective study to examine the effect of various diagnostic tools on the staging or management of cancer in multiple health systems in the Seattle-Puget Sound area. A protocol will be developed and presented to our health system partners for input prior to the end of the second year.

ADVICE continues to implement two retrospective CER studies of advanced imaging in cancer: Study #1: MRI and breast cancer staging, and Study #2: PET vs. traditional imaging for lung cancer staging and follow-up. Study #1 Update: We completed variable coding, ran the analyses, and drafted a manuscript on imaging patterns. We also prepared a dataset for an analysis on treatment paths. This analysis uses different time definitions which required going back to the original data. We expect to complete a treatment path analyses and a draft manuscript in the next couple months. We completed a preliminary provider and clinic analyses as requested by our stakeholders. This required developing a cross walk for providers. We were not able to do this for all plans due to unique coding systems. We also rolled up providers into clinics. We found tremendous variation in the use of advanced imaging and in the average payment per patient. Data were presented to Stakeholders on July 24th and to Advisory Board members on August 2nd. Both groups were very interested in this variation and wanted us to drill down further to better understand why such a large variation existed. We are currently exploring ways to continue this work beyond ARRA funding. Study #2 Update: We received claims data from participating plans for all matched SEER lung cancer cases and completed pre-analytic data cleaning. We would like to complete a high level analysis on imaging (similar to what we did with breast cancer), repeat the VA's futile thoracotomy approach with the full ADVICE database, and conduct a provider analyses on lung cancer cases, but are not sure what will be possible. We need additional funding and staffing to complete cleaning of the raw files. Once this is done we have several investigators with plans ready to mine the data. |
