Grant Name: Captial Improvment Program, New Facility Project Caring Health Center, Inc, (CHC) is an integral part of the community health care system in greater Springfield (Massachusetts) with a proven history of addressing issues that affect the health of community residents in an area that is medically underserved. CHC offers community services at a number of locations in Springfield with limited space. In November 2007, CHC acquired a 45,200 square foot, nineteenth century building in the South End of Springfield. We plan to renovate the three conjoined historical buildings to expand services. This expansion will allow CHC to upgrade and expand its community health services to address the pressing medical, dental, nutritional and behavioral health needs of poor, unserved and underserved residents of Springfield. Prior to renovation, plans must be approved by the Massachusetts Department of Public Health. The purpose of this grant is to secure an architectural firm to develop the detailed plans necessary for licensing approval, in preparation for construction on our new facility. Dietz and Company Architects, Inc. is the firm selected to perform this task. During this grant period, the goals are to complete investigation, programming, schematic design, demolition and abatement, design development, and preparation of construction documents. Achieving these goals is essential and necessary before construction can begin. Without this work the project will not be able to proceed and CHC will not be able to meet the needs in our community effectively due to the current space constraints.

Project Activities Description Ambulatory & Primary Health Care
Quarterly Activities/Project Description Caring Health Center (CHC) submitted an application for Capital Improvement Program funds in 2009. The purpose of our project was to secure an architectural firm to develop the detailed plans necessary for licensure approval, required before we could begin construction on our new facility. CHC has complied with all Terms and Conditions of the grant and has successfully satisfied all of the submittal requirements listed in the guidance. Caring Health Center requested an Extension Without Funds when it became clear that all funds would not be drawn down due to a delayed construction start date. Construction has begun for tornado damage repair. All design development funds for the project have been drawn down. All other work proposed in CHC?s grant application for architectural detail design development plans necessary for licensure approval is complete. The last and final sets of plans were submitted to the Department of Public Health on August 31, 2011, for final approval with an expected turnaround time of November 2011. The design development and the detailed architectural plan design for the project is 100% complete. In an effort to finalize the project financing, CHC continues to meet with its finance team on a regular basis. Due to unforeseen setbacks (Tornado), construction for the actual ?New Facility Expansion? project is not expected to begin until late December 2011 or early January 2012.
